With a PPO plan, you can visit both an in-network or an out-of-network dentist of your choice. PPOs are typically subject to the following or similar terms and conditions:

  • Deductible – This is the amount of money you’ll need to pay out of pocket before your plan becomes available for any treatment beyond your checkups, cleanings, and regular X-rays.
  • Annual Maximum – This is the specific dollar amount of coverage Humana will provide you and your dependents within a calendar year.
  • Waiting Period – For some Humana plans, you may have a waiting period before your coverage becomes fully available. The exact amount of time will vary depending on your individual policy. Thankfully, this waiting period does not typically apply to preventive exams and cleanings.

Be aware that when you select a PPO plan, you’ll likely pay a higher premium, but this typically translates to more favorable terms. In most cases, you can expect your coverage to follow similar guidelines:

  • 100% of your costs for two exams and cleanings each year.
  • 80% of the total for “minor” dental procedures.
  • 50% of the total for “major” dental procedures.

Please note that the exact definition of major and minor procedures varies by provider, making it extremely important to verify your coverage before committing to treatment.

HMO plans require you to select a primary, in-network dentist, and Humana will only pay for claims with a dentist that is in-network. However, HMO plans typically have lower premiums than PPO plans and are best for patients who are in good oral health and don’t anticipate needing any major dental procedures in the future.
